When you are applied to Handicapped Person's Welfare Law Article 15-designated doctor newly, you attach copying of written consent, application, resume, doctor driver's license, and please apply.
※Please submit new application by last month 20 in holding month to examine by subcommittee, examination sectional meeting specialized in council for Saga social welfare person with a physical disability social welfare held four times a year (May, August, November, February). As it may become examination by the next holding when it is past time limit, please be careful. (e.g.,: in November for holding on → October 20 on the application deadline)
About notification of change
The following case, please submit notification of change.
- When the office is changed to another medical institution in the prefecture
- When we run business newly in the prefecture
- When name of working medical institution or the location is changed
- When full name is changed

- When it is worked at medical institution of outside the prefecture
- When Article 15-designated doctor is declined
- When it is died
